At first, the journalists weren't looking for this perspective. But then many whistle-blowers spoke up, and their information was very concerning.

To us, this is a telling example of an organization's structural problems that create human factors, which can lead to gruesome consequences.

What's worse is the issue was known: in February and April of this year, the US Armed Forces suspended Boeing's KC-46A deliveries after FOD and debris was found inside the aircraft's structure (read more here and here).
